Insurance
All CSB participants are insured through Global Secutive, with UnitedHealthcare being the claims administrator. The Health Plan with UnitedHealthcare is: UnitedHealthcare Options PPO / Policy 1190028 / Phone: 1-800-251-1712. In most cases the doctor or the hospital will directly bill UnitedHealthcare, meaning the participants will not need to submit a claim form directly. If you are a current program participant, we recommend that you always visit the Global Secutive website www.eSecutive.com/MyInsurance. To set-up your personal account at MyInsurance (tab "Create an account") you will need to enter your last name, first name, policy number and date of birth. Once you have completed your registration, you will receive a confirmation email with your login information. Please keep this confirmation safe. Once logged in, you will have access to detailed information pertinent to your insurance including:
- Emergency Service Hotline Numbers
- Provider (Doctor of Hospital) Search
- How to file a claim instructions & Claim form
- Copy of your certificate
- New ID card
- Insurance conditions
- Insurance brochure
- Live Chat with a Service Representative
- Online Claim Status Check
Attention: If a participant is injured at work, CSB must be notified immediately. In such case, all claims should be referred to the employer's worker's compensation policy.
